S.M.A.R.T. Goals

These goals were set within the context of my next clinical experience as a pre-consolidation student at an outpatient diabetes clinic in the Summer of 2026.

Clinical Decision-Making

By the end of my placement, I will be able to independently analyze assessment findings, laboratory results, and blood glucose trends for at least 10 patients and collaborate with my preceptor to develop evidence-informed nursing interventions.

Health Promotion and Prevention

By the midterm evaluation of my placement, I will be able identify risk factors for diabetes-related complications in at least 5 patients and provide appropriate education and referrals to promote complication prevention.

Chronic Disease Self-Management

By the end of my placement, I will support at least 10 patients in setting realistic self-management goals related to glucose monitoring, medication adherence, nutrition, or physical activity, and evaluate progress using teach-back and follow-up discussions aligning my teaching strategies to their health literacy, culture, and readiness to learn
